A patient, or his/her legal representative, may inspect and/or obtain a copy of their medical records, or have copies of medical records sent to another facility.
Stanford Health Care requires a completed and signed Authorization for Release of Health Information form before releasing any documents to anyone, including the patient. In certain cases, a patient's physician, psychologist or social worker may also be required to approve a request made using a release form.

There are two ways to request a copy of your medical records. You can either submit a paper form or request a copy online using your MyHealth account. Instructions for both are below:

Using MyHealth to View/Download/Export Medical Images
Patients can use MyHealth to retrieve, view, and download their radiology images if the imaging exams were performed at Stanford Health Care. Images from an exam performed outside of Stanford Health Care will not be available to view in MyHealth. To learn more, please see our FAQ.
